Program: The Crisis Response Team (CRT) is dedicated to providing quality care and assessments to individuals and families who are in need of crisis intervention. The specially-trained staff oversees emergency walk-in services and crisis calls for the Weld County area and after-hours emergency on-call services for a 12-county Northeast Colorado region. Consumers can expect to have a thorough assessment of their needs and situation upon contacting our staff who assists in determining what level of care would most benefit the individual in the least restrictive setting. CRT also provides intensive care coordination for Weld County clients and discharge planning for clients stepping down from higher levels integrating back in to our communities.
Participates as a member of specified program team to implement the individual care plans for clients with mental health and/or substance use challenges in accordance with the applicable policies, rules, regulations and directives of the organization. Facilitates client participation in their care plan by interaction with them as a source of emotional support, resource problem solving, and advocating for their needs. Assures security for clients by monitoring and supervising the facility and the services delivered by the facility.
– Bachelor’s degree in psychology, social work, or other behavioral health discipline from an accredited college or university required
– Medication Aide Certification required (QMAP) (training offered post hire)
– Valid Colorado Driver’s License and a driving record that meets with hiring requirements as set forth for someone that must transport customers
– No experience necessary but is preferred
– Demonstrated ability to plan and use effective interventions with customers

– Plan, organize, time manage and prioritize multiple tasks and assignments.
– Work with diverse populations and be culturally sensitive.
– Work as a team member, maintain effective working relationships, and foster a positive and cooperative work/residential environment.
– Perform tasks with a focus on exemplary customer service.
– Maintain confidentiality.
– Assist supervisory staff in assuring that clients comply with program rules, guidelines and directives while they participate in all aspects of daily living and treatment within the facility and outside the facility when instructed.
– Assist in the client admission process including orientation, training, and other activities to maximize clients’ positive adjustment to the setting.
– Observe client behavior to identify change or indicators of special needs and report those observations to designated staff as they occur.
– Participate in the delivery of group care activities and serve as assistant to the clinical staff in treatment events as requested. May participate in one-to-one treatment sessions with clients as designated by program.
– Assist with meal service and participate with clients during the meal time as indicated by client care plans; assist with client wellness checks and breaks per program requirements. May be required to monitor clients on breaks where tobacco products are used.
– Conduct house/community meetings and engage clients in discussion.
– Assist in monitoring client use of medications and the management of those medications.
– Transport clients using company vehicles when instructed to do so by supervisory staff for routine and/or emergency/crisis events.
– Assist in special outings, tours, trips and other special projects that are part of the program plans.
– Function as crisis intervention specialist when required to implement response plans for a client demonstrating difficult behaviors, for health emergencies, or for natural disasters.
– Use appropriate de-escalation, seclusion and/or restraint techniques when necessary in accordance with program guidelines.
– Demonstrate competency in use of time outs, token economy, therapeutic privacy, therapeutic physical restraint, and locked time out as behavioral interventions as appropriate for the facility and situation.
– Function as a member of the facility safety team by assisting in the monitoring of compliance with all safety regulations and by participating in the ongoing orientation and training of clients in all emergency response procedures (i.e. fire evacuation).
– May have on call or emergency services assignments.
– Follow all state and local requirements.
– Assist in client relationships with family and/or other key individuals, in a manner consistent with the client’s care plan. May communicate with family members or guardians concerning home visits and/or discharge plans.
– Maintain manual and electronic clinical and other operational records as required; confirm coding of services rendered is ethically appropriate and results in successful billing.
– Demonstrate fiscal responsibility by supporting agency collection policies and procedures; assure client understanding of service charges and fee changes.
– Work proactively with payor sources to ensure payment of services; align services with payor authorizations and/or request authorization prior to provision of service.
– May perform laundry responsibilities as assigned.
– May provide drug monitoring services and/or take client vital signs (i.e. pulse rate, blood pressure, body temperature) per program assignment.
– Perform other duties as assigned.
Services may involve travel to offsite locations for assessments and crisis management, including transportation of clients. Work hours can vary and may require weekends, evenings, holidays, and/or on-call scheduling. Residential assignments may require on-duty meal periods.
